Adults

Adults, or those individuals over the age of 18, will click this link and complete the attached forms.

All forms MUST be completed by the client enrolling in services as these are legal documents.

Children

Children, up to their 13th birthday, must have their parent or legal guardian's consent and agreement for financial responsibility to enroll in services. The legal guardian MUST sign the documents.
​A parenting plan may be requested.

**We are not currently accepting young children due to the inability to see them in person. We apologize for this inconvenience.**

Teens

Teens, or those individuals 13 years old until their 18th birthday, must consent to their own treatment but are not able to sign financial contracts. Teens must be willing to sign a release of information allowing us to speak with their legal guardian or the individual agreeing to be financially responsible for them. This individual must sign the financial responsibility form, and all consents for treatment are signed by the teen.

The teen MUST be the individual who starts and completes this process, including signing legal consents for treatment.
